Input a code of up to four characters that represents the restriction reason.

ClosedRestriction Type*

Select from the dropdown the type of medical restriction from a return to work agreement:

Code Description
BENDING Bending Restriction
REACHING Reaching Above Head Height Restriction
STANDING Standing Restriction
TWISTING Twisting Restriction
WEIGHT Weight Restriction
